Note: The Corporation may accept KYC documents even if there is a change in name subsequent to the issuance of such KYC Document provided that the same be accompanied by a self attested copy of the a Marriage Certificate issued by a State Government or a Gazette Notification indicating such a change in name, as the case may be

1 A declaration for communication address in the prescribed format
AND Any one of the following documents can be accepted
2 Photo ID card containing the address, issued by any of the following organizations/Institutions
  1. Central Government or any of its Ministries.
  2. Statutory / Regulatory authorities
  3. State Govt. or any of its Ministries
  4. Public Sector Undertaking (established under GOI or State Govt)
  5. State Govt. of J&K (Id card issued by the State Govt. of J&K is acceptable if they carry the connotation - 'Government of Jammu & Kashmir, Civil Secretariat, Srinagar/Jammu')
  6. Bar council
  7. Senior Citizen card issued by State/Central Govt.
  8. Govt. of India to Persons of Indian Origin [PIO card], OCI Card
  9. Defence Dept / Ministry of defence for Defence personnel & their dependents
  10. Public Financial institutions/ Public Sector Banks
  11. Identity card issued by public companies listed on recognized stock exchanges in India with Employee number mentioned on the said identity card.
  12. Scehduled Commercial Bank
3 Ration Card. (Not to be accepted as address proof in the States where the Government/ High Court has notified that ration card cannot be accepted as address proof. For eg. Maharashtra)
4 Demat account statement issued by a registered depository participant clearly stating the address, not more than 3 months old.
5 Copy of title deeds, clearly indicating that the original title deeds were duly registered and stamped or copy of 24 Land Patta / Jamabandi / 7/12 extract.
6 Permanent Account Number (PAN) [PAN/ TAN allotment letter stating the address, subject to the copy of the PAN/ TAN card also being submitted]
7 Letter issued by Employer
8 Copy of the Lease / Leave & license agreement / Rent agreement duly stamped/ registered as per state laws, not less than 3 months old. If any of the said agreement is less than 3 months old, then the customer may submit a copy of the utility bill issued in the name of the landlord or in case there is no change in the address as stated in the earlier agreement, the customer can submit copy of any of the earlier agreement duly stamped/ registered as per state laws. (other than cases covered under point no. 14 above)
9 Additional Address Proof: Defence Personnel (Indian Army, Navy & Air Force) Service Certificate issued by the Defence Ministry / Unit of Defence Ministry mentioning the name and address of the applicant Or Duly attested and certified by the commanding officer of their unit/s in the format
* The above documents may also be accepted in case of Retired Persons/Pensioners/Housewives/Unemployed persons

In case the prospective customer is a Non-Resident Indian (NRI)/Person of Indian Origin (PIO)/Overseas Citizen of India (OCI), the KYC documents are to be self attested and submitted at the time of application. Though the KYC documents of the Power of Attorney holder can be verified with the originals, it becomes difficult to verify the documents submitted by the prospective NRI/PIO/OCI Customer with the respective originals where he / she is not personally present at the time of application / disbursement process. In such a case, it is suggested that any one of the following documents be obtained -
1 a.  Proof of identity and address duly notarized by the Notary Public (overseas) having authority in the place of residence of the prospective NRI customer.
2 b.  Proof of identity and address duly attested by the Indian Embassy existing at the place of residence of the prospective NRI customer.
